Aside from all the state and federal deductions you will also need a space in which to put any deductions that have to do with your business, things such as health insurance, retirement plans and health savings accounts are pretty standard and should be included on any blank pay stub template that you acquire. You should also have a single slot for miscellaneous deductions such as any loans or equipment that your employee may owe you for.
Finding a blank pay stub template that has everything in it that you need should not be hard, in most cases you can download a free one off the Internet that should include everything on it that you need.
